Summer School and ESY families,
As we move away from the traditional school and closer to “normalcy” in regard to the public health situation in our state and community, we will no longer be requiring families of students to certify COVID-19 close contact and symptom-free acknowledgements via a daily form.
Rather, all students who will be attending any Huntley 158 facility June and July 2021 will be required to certify once via the form below. Please note that the form below, while completed only once, signifies that you will continue to self-verify on a daily basis your student’s eligibility to enter District buildings and your compliance with public health guidelines.

Please complete the certification form for June/July 2021 prior to attending any learning or other in-person experience on or after June 7, 2021. This form will need to be completed separately for each student in your family who is attending summer programming.
Health screening requirements for ongoing IHSA athletics will continue to follow existing procedures, and requirements for athletic camps over the summer will be communicated by camp organizers.
