Dear Parent/Guardian of a 9th Grade Student,
We hope that you are enjoying your summer. Unfortunately, summer will be over before you know it. With the beginning of school less than 2 months away, we are reminding all parents of incoming freshman to submit the required medical paperwork if you have not done so.
All students entering 9th grade must have a current school physical exam and immunizations submitted to the health office prior to material pick up days on August 16th and 17th.
A sports physical does not fulfill the requirements mandated by the state. A student without a physical on file will be unable to pick up his/her class schedule or other materials. There are no exceptions, and an appointment date will not be accepted in lieu of a physical.
Completed physicals can be submitted to the high school office, scanned to the email addresses or faxed to the fax number below. Physicals may also be hand carried to the nurses on material pick up days for clearance, though this may slow your progress on that day. If you have questions, you can email us or leave a phone message. We will be available in the health offices sporadically for the remainder of the summer and will respond when able.
